While requesting again , you need to enter your Order ID which you have got while ordering Olay sample or alternatively you can get it in your pgimprovinglife.com "My account "section
UPDATE: Just Got an Email from P&G Representative saying that they have closed this form till further notice and now you need to send an email to info@pgimprovinglife.com to know about the status of your delivery.
